Referral for Review Affidavit

The Referral for Review Affidavit must be sworn prior to being filed with the Violation Ticket Centre. Your Fare Infraction Ticket and the decision letter from the arbitrator to the affidavit should accompany this form which must be signed by a Commissioner within 30 days of receiving your notice of decision. http://www.th.gov.bc.ca/TransLinkDispute/index.html
