All relevant sections must be completed by both transferee and transferor and submitted along with all supporting documentation (detailed in the information requirements section of the application package) before your application can be processed.
The PT5002: Signing Authority - (37 KB PDF) is a single page document which applicants may use when specifying the list, or change to an existing list if one is already on file at the Passenger Transportation Branch, of everyone authorized by the applicant to correspond with the Passenger Transportation Branch and/or the Passenger Transportation Board regarding passenger transportation matters. The PT5005B: Vehicle Safety Compliance Declaration – (91 KB PDF) is a 1 page document which you (transferee) may use to declare that vehicle(s) you have purchased, leased or rented for commercial passenger use meet the provincial and federal safety requirements.
