What if my Special Authorization Vehicle Identifier (plate and/or decal) is lost or stolen?

If your assigned Special Authorization Vehicle Identifier (plate and/or decal) has been lost or stolen, you will need to include the following information with your request for a replacement:

  • Payment of the $50 replacement fee.
  • Completed PT5018: Declaration of Lost or Stolen late and/or Passenger Transportation Document. A copy of this form can be obtained from the Passenger Transportation Branch or downloaded in the Existing Licensee Forms section below.
  • A Photocopy of the vehicle registration (e.g. “Owner’s Certificate of Insurance and Licence/Vehicle Registration”).
  • A photocopy of the Commercial Vehicle Inspection Report, form MV3104 of the Commercial Vehicle Inspection Program (CVIP), that verifies that the vehicle has been inspected and passed by an approved vehicle inspection facility; Or equivalent, if the vehicle is not registered in British Columbia. Inspections are required to be performed on a semi-annual basis unless your operation is part of a preventative maintenance program.

What if I need to replace my Passenger Transportation Licence?

If your Passenger Transportation Licence has been lost or stolen, you will need to include the following information with your request for a replacement:

  • Payment of the $50 replacement fee.
  • Completed PT5018: Declaration of Lost or Stolen Plate and/or Passenger Transportation Document. A copy of this form can be obtained from the Passenger Transportation Branch or downloaded in the Existing Licensee Forms section below.

What if I need to replace my Special Authorization Vehicle Identification Certificate (VIC)?

If your Vehicle Identification Certificate has been lost or stolen, you will need to include the following information with your request for a replacement:

  • Payment of the $50 replacement fee.
  • Completed PT5018: Declaration of Lost or Stolen Plate and/or Passenger Transportation Document. A copy of this form can be obtained from the Passenger Transportation Branch or downloaded in the Existing Licensee Forms section below.

How can I transfer my existing Special Authorization Vehicle Identifier to a new Vehicle?

Special Authorization Vehicle Identifiers (plates and decals) are vehicle specific. When a vehicle is no longer in service and is being replaced with another vehicle it is the responsibility of the carrier to inform the Passenger Transportation Branch of the change. You will need to include the following information with your request to use an alternate vehicle:

  • Payment of the $50 Identifier Fee for each request.
  • Completed PT5019: Application to Place Vehicle Identification Certificate and Accompanying Identifiers (Plate & Decal) on Alternate Vehicle. A copy of this form can be obtained from the Passenger Transportation Branch or downloaded in the Existing Licensee Forms section below.
  • A Photocopy of the vehicle registration for the new vehicle (e.g. “Owner’s Certificate of Insurance and Licence/Vehicle Registration”).
  • A photocopy of the Commercial Vehicle Inspection Report, form MV3104 of the Commercial Vehicle Inspection Program (CVSE), that verifies that the vehicle has been inspected and passed by an approved vehicle inspection facility; Or equivalent, if the vehicle is not registered in British Columbia. Inspections are required to be performed on a semi-annual basis unless your operation is part of a preventative maintenance program.
  • If the new vehicle is wheelchair accessible, you must include a photocopy of the Vehicle Inspection Report (CVSA), form MV3110 of the Commercial Vehicle Inspection Program (CVSE); or equivalent if the vehicle is not registered in British Columbia; or a clear photograph of the secondary manufacturer label displaying the Canadian Motor Vehicle Safety Standards (CMVSS) or the Federal Motor Vehicle Safety Standards (FMVSS) approval.
  • If the new vehicle is registered outside of British Columbia, the vehicle must be registered in the name of the licensee and you must include a photocopy of a current & valid British Columbia Financial Responsibility Certificate (BC Insurance Filing number) issued by the Insurance Corporation of British Columbia (ICBC); OR

    A photocopy of a current valid British Columbia Financial Responsibility Certificate (BC Insurance Filing number) issued by the Insurance Corporation of British Columbia (ICBC); OR

    If you do not have a BC Insurance Filing number include a current written explanation on how you are intending to meet your motor vehicle liability insurance requirements (e.g. you will purchase a non-resident single commercial vehicle permit (NRSCV) or a non-resident quarterly commercial vehicle permit (NRQCV), that includes an insurance certificate, from the Provincial Permit Center’s toll-free vehicle permit line: 1-800-559-9688 or at an Inspection Station, formerly known as a weigh scale, when entering British Columbia).

  • If the new vehicle is licensed outside of British Columbia you must include photocopy of your apportioned cab card showing proof that the vehicle is registered in British Columbia with the International Registration Plan (IRP);
    OR
    If you are not registered with IRP, include a written explanation on how you are intending to pay your British Columbia vehicle licensing fees (e.g. you will purchase a non-resident single commercial vehicle permit (NRSCV) or a non-resident quarterly commercial vehicle permit (NRQCV), from the Provincial Permit Center’s toll-free vehicle permit line: 1-800-559-9688 or at an Inspection Station, formerly known as a weigh scale, when entering British Columbia).

How can I request changes to my carrier file?

As an existing Licensee, you are invited to use any of the following forms to assist in requesting updates to your application file. Completed paper forms, along with all supporting documentation, must be submitted to the Passenger Transportation Branch.

  • The PT5002: Signing Authority - (37 KB PDF) is a single page document which applicants may use when specifying a change in the list of everyone authorized by the applicant to correspond with the Passenger Transportation Branch and/or the Passenger Transportation Board regarding passenger transportation matters. Please note specimen signatures must be included for everyone listed in this document.
  • The PT5009: Notification of Change of Address - (20 KB PDF) is a single page document which applicants may use to inform the Passenger Transportation Branch of a change to their business address.

How can I make a payment to the Passenger Transportation Branch?

All payments must be made in Canadian Funds payable to "Minister of Finance". Payments can be made by cheque, money order, debit, Visa or MasterCard.
