End Product Specifications (EPS)
Paving Quality Acceptance and Quality Control Forms

Ministry Forms

The Quality Acceptance and Field Lab Reports are Excel spreadsheets developed by the Ministry Field Services and are meant to be used by the Ministry Representative on End Product Specification paving projects.

The Quality Acceptance Reports are to be used in conjunction with The Ministry of Transportation's End Product Specification Appendix 502 EPS Version 6.0 (February, 2003).

The Quality Acceptance Reports are used to determine acceptance and/or rejection of asphalt pavement produced by Contractors working on End Product Specification paving projects.

Quality Acceptance Forms and the Field Lab Report, provided by the links below, should be downloaded, filled in, and printed out by the Ministry Representative, and then provided to the contractor:

This Quality Acceptance Report contains:

  • Segregation Payment Adjustment
  • Lot Assessment and Payment Adjustments for Smoothness
  • Lot Adjustment Summary: Segregation and Smoothness

EPS-FLR.xls

This Field Lab Report contains:

  • Degree of Compaction from Test Cores
  • Mechanical Analysis of Aggregates & Determination of Asphalt Content
  • Mechanical Analysis of Aggegrates
  • Ignition Oven Calibration
  • Field Lab Summary

Contractor Forms

The Quality Control Form is an Excel spreadsheet jointly developed by the Ministry Field Services and BC Roadbuilders Association, and is meant to be used by the paving contractor on End Product Specification paving projects.

The Quality Control Form provided by the link below, should be downloaded, filled in, and printed out by the contractor, and then provided to the Ministry Representative:
